Fill out the student registration form below after you have received confirmation about your payment. It is important to fill out this form to ensure you are part of the enrolled students’ database.

You’ll know you have successfully submitted this form if you get redirected to the Thank You page. Next, check your email for an email from us and follow the prompts to confirm your subscription to our mailing list. This is a CRUCIAL STEP as this will ensure you will get updates from Review Masters Bicol – especially updates meant for enrolled students.

This is the form for PAID students (both FULLY and PARTIALLY paid). Fill out this form if:

  1. You have paid, AND We have verified your payment.

    2. This is also for you if you enrolled and paid in person via our enrollment/registration booth.

  • If you filled out any of the forms in our website before (the slot reservation form, or the purchase/enrollment form) and have confirmed subscription to the Review Masters Subscribers List, please ensure to use THE SAME EMAIL ADDRESS you used before when you fill out this form so that your past records can be successfully merged with these new records.

  • Please enter only the Municipality/City and Province name. Do not include street, barangay, and other address information.

  • College Plans

  • Preferred Courses/Programs

  • List down, in order of priority, your preferred courses/programs. Write at least 4 courses.

  • College Entrance Tests (CETs) you plan to take and are preparing for

  • List down ALL the college entrance tests that you plan to take. Examples: UP College Admission Test (UPCAT), Bicol University College Entrance Test (BUCET), Ateneo College Entrance Test (ACET)

  • Enrolment Information

    • Numbers only.
    • The Order # you were given upon online enrolment or slot purchase. Example entry: 6847.
    • If you are officially part of a group that enrolled online, ask the member who processed your online enrollment for your Order #.
  • Does your Order # pertain to an Individual or a Group Enrolment?
  • Choose the venue you (or your group) enrolled. In case your entry here differs from what is in your online enrollment (i.e., online slot purchase), your online enrollment record will prevail.
