After paying – partially or in full – by bank deposit , please fill out the Student Registration Form for Enrolled Students found at http://www.upcatreviewplus.com/register/.
You’ll need to have a head-shot photo of yourself and a picture of your Proof of Payment (e.g your Bank Deposit slip or Transfer to Anyone confirmation or Acknowledgment Receipt) to use the form. The photo and your proof of payment picture should preferably be no more than 1MB in size and must be in any of the accepted file formats: .jpg, .jpeg, .png, .pdf.
NOTE: THIS STEP IS TO BE ACCOMPLISHED AFTER BALANCE COMPLETION (i.e. for those who PAID PARTIALLY WHEN THEY ENROLLED). IF YOU PAID IN FULL WHEN YOU ENROLLED, FILL OUT THE STUDENT REGISTRATION FORM FOR ENROLLED STUDENTS – BUT ONLY IF YOU HAVEN’T DONE SO ALREADY.
After paying your balance via bank deposit, you will need to fill out a second form: the Payment Completed Form, which you can find at http://www.upcatreviewplus.com/payment-completed/.
